Arrive prepared for a smoother visit

If your doctor provided you with an order for a test, you will need to visit one of our more than 2,250 Patient Service Centers to have your specimen collected.

Get a lab order from your doctor

Your doctor may prescribe lab testing as part of your visit. Or you can request a specific lab test from your doctor. A lab order can be printed or digital. If your doctor doesn’t hand you a printed lab order, we recommend contacting them prior to your Quest appointment to confirm that it was sent electronically.

Alternative option: Purchase your own test

Another option is to buy the test for yourself. It's a simple way to get the test you need, without a doctor's visit. Shop for tests now.

Make your appointment

Minimize your wait by making an appointment. Appointments take priority over walk-ins. Walk-ins may be accommodated during the next available appointment opening. Scheduling an appointment will also give you the option of getting helpful pre-appointment email reminders and tips from Quest. If you prefer to walk in, it helps to know that our Patient Service Centers are usually busiest in the early morning.

Know before you go

Bring the following to your appointment:

  • Lab order from your doctor: If your doctor did not give you a paper order at your appointment, check if they sent it electronically.
  • Photo identification
  • Current health insurance information
  • Credit, debit, or health savings card: You will be asked to preauthorize payment for any balance due after your insurance is billed.

It’s important to also know if your test requires fasting the day before and/or morning of your appointment. Your healthcare provider should have indicated this on your lab order or during your visit! Learn more about fasting before a lab test.

Paying your bill

If you're insured

Easy Pay

You will need to provide a credit, debit, or health savings card during your Quest Diagnostics visit. You’ll authorize an amount based on the estimated out-of-pocket cost of your lab test—but we’ll only charge your card if there’s a patient cost after we bill your insurance company. We don’t hold your funds and won’t charge you if your insurance pays the full amount. Learn more about Easy Pay™. Whether you’ll owe a portion of lab testing completely depends on your insurance. Your responsibilities typically include copays, deductibles, coinsurance, and any testing services not covered by your plan. If you have questions about your coverage, contact your insurance company.

Patient Assistance Program

We also offer payment plans. If you need help paying for the tests you need, our Patient Assistance Program may be able to help.

Advance Written Notice (AWN)

During your appointment, you may be asked to sign an Advance Written Notice (AWN). Health plans do not cover all healthcare services. In certain cases, for tests that Quest Diagnostics knows are likely not covered based on a health plan’s policies, we’ll issue you an AWN to notify you of your potential financial responsibility, and we’ll ask that you review and sign it if you’d like to proceed with the service(s). Patient Assistance Program

Uninsured patients are required to pay for their testing at the time of service. Ask your doctor about our discounted pricing for uninsured patients. If you need help paying for the tests you need, our Patient Assistance Program may be able to help.

Frequently asked questions about lab testing

Unless otherwise noted, the answers to the following questions apply to tests ordered by healthcare professionals only.

Requesting lab tests

We are the world's leading provider of diagnostic testing, information, and services. Our clinical laboratory testing services include: blood tests, body fluid testing, tissue pathology and cytology, health screening and monitoring tests, drug screening and testing as well as gene-based testing (genetic testing). Visit our testing page to see a list of frequently ordered lab tests and learn why and how laboratory tests are done, and what information your doctor can obtain from the results.

You can select and pay for lab tests here. Licensed professionals from PWNHealth LLC and affiliated professional entities (collectively, PWN) will provide clinical oversight of your lab testing, including ordering your selected tests, when appropriate. Then, have your specimen collected for the testing you purchased. Quest offers testing you can purchase for yourself, with physician oversight, as follows:

  • Tests with an In-Person at Quest or In-Person at Drive-Thru option can be purchased in all states except Arizona, Alaska, and Hawaii, as well as Puerto Rico.
  • Tests with an At-Home Kit option can be purchased in all states except Arizona, New York (with the exception of the COVID-19 Active Infection Test), Alaska, and Hawaii, as well as Puerto Rico.

Please note that New York residents may only purchase an at-home kit for a COVID-19 Active Infection Test. New York residents may not purchase any other at-home kits.

We strongly recommend that you share your test results with your doctor in order to make the best health decisions.

Preparing for a lab test and/or visiting a Patient Service Center

Find a convenient Quest location. The address, hours, phone number, directions (map), and services provided are listed for each location.

Appointments are strongly encouraged and take priority over walk-ins. We offer online appointment scheduling for individuals who like the ease of scheduling at a location, date, and time that is convenient for them. You can see which locations offer appointments by accessing our online scheduler. You also have the option of downloading our appointment scheduling app to your smartphone.

Find the location you are interested in visiting to see the days and hours the location is open, maps, and driving directions.

Some blood tests do require fasting prior to having your blood drawn. Please contact your healthcare provider to determine his/her requirements for your specific testing.
