OUR PROGRAM

Better Food for Better Health

Pack Health has combined its one-of-a-kind Better Food for Better Health (BFFBH) program with Instacart® to support individuals with chronic conditions who are also experiencing food insecurity.

 

This program includes:

Weekly one-to-one member engagement via phone calls, text messages, and emails

Members use a predetermined dollar amount to select fresh foods from a preapproved list - delivered straight to their home via Instacart®

Recipes to effectively utilize the provided healthy, fresh foods

Support to manage comorbid conditions and the impacts of social determinants of health
